The first job in any tracking system, whether it is a notebook, a spreadsheet, or software, is an honest inventory. Licenses accumulate over a career, and each one was tracked somewhere different at the time. This worksheet gives every credential its own row with the fields that matter: who issues it, when it expires, how long the cycle runs, what CE the board expects, when the renewal window opens, and when you last confirmed the record against the issuing authority.

Fill in one row per credential and per state, not per profession, because two boards in two states rarely share a cycle or a CE rule. Leave passwords out of this sheet entirely and keep those in a password manager. The example rows below show the kinds of credentials people forget; replace the placeholders with your own details, and put today's date in the last column only after you have checked the board's own record. If you manage a team, the Firm License Tracking Time Savings Calculator at /tools/firm-license-tracking-savings/ shows what keeping this sheet current across a whole firm is worth.
